Sealed bids will be received by the PALM HARBOR COMMUNITY SERVICES AGENCY, INC., at the Community Room of Palm Harbor Public Library, 2330 Nebraska Avenue, Palm Harbor, Florida 34683, until NOVEMBER 20, 2007 at 2:00 P.M. and will then and there be opened and read aloud for the following:

All Services, Labor, Materials and Equipment required for Interior Renovation of:
PALM HARBOR PUBLIC LIBRARY

The estimate for this project is $1,000,000.00, including Owner-furnished moving expenses that are not in the general contractor's scope of work.

All bids for construction services will be considered only from bidders who are currently pre-qualified with Pinellas County Board of County Commissioners Purchasing Department for General Construction that equals or exceeds their bid amount. Such prequalification category and limit must be approved prior to the bid opening date. Any bids received after the specified time and date will not be considered.

Specifications and plans are available for pick-up at the Palm Harbor Library by contacting Debbie Phillips, Palm Harbor Library Administrative Assistant, at 727-784-3332, extension 216. A charge of $50.00 (Fifty Dollars) will be made for each set. Please note that postage or courier expenses plus a $25.00 per order handling fee will be charged for all plans and specifications sent by mail or courier. No refund will be given for the bid documents.

A non-mandatory Pre-Bid Conference will be held at the Palm Harbor Public Library on November 13, 2007 at 3:00 P.M.

ADDENDUM NUMBER ONE
General Contract Bidders are reminded, in accordance with the Instructions to Bidders, that only those bids from Bidders who are pre-qualified with Pinellas County in the amount that equals or exceeds their bid prior to the opening of bids will be considered. This is a pre-qualification project requiring Bidders to be prequalified for General Construction.

The following modifies, revises or clarifies the Plans or other Contract Documents as follows. The Bidder shall acknowledge that his proposal is based on the plans and specifications as modified herein by indicating same in the space provided on the Proposal and Bid form.

Item 1.01 Instructions to Bidders, Paragraph 13 - Definitions, Owner: Change to read: "Owner - This project is owned by the Palm Harbor Community Services Agency, Inc., a Florida Not-for-Profit Corporation; 1500 16th Street, Palm Harbor, Florida 34683, hereinafter referred to as the County or the Owner."

Item 1.02 Instructions to Bidders - Proposal and Bid, Alternate 1: Carpet types: Add, "Bidders shall include the cost of carpet removal in the Base Bid. The intent of this Alternate is to allow the Owner to select either one or the other carpet products offered by Interface FLOR or Milliken. The Owner reserves the right to reject both Alternates."

Bidders shall provide two prices for the carpet alternates on the Proposal and Bid form as follows:

Item 1.03 Construction Agreement - Exhibit J: Asbestos Survey: A NESHAP Interior Renovation Asbestos Survey has been performed by Greenfield Environmental, Inc. No asbestos was detected in the samples taken. A copy will be made available for review by bidders at the pre-bid conference.

Item 1.04 Drawing A-201, Entry 101: Delete references to Hurricane Netting Shutters at Entry 101. A metal removable hurricane shutter with surface-mounted track and recessed wall and floor anchors will be provided by the General Contractor. Specifications will be issued with Addendum No. 2.

Item 1.05 Drawings A-201 and A-802, Entry 101 and covered entry at doors 25 and 26: Carpet walk-off matt is as specified on Drawing A-903 Flooring Schedule and Specification 12484: InterfaceFLOR, Pattern "12901 Entry Level."

Item 1.06 Drawing A-903, Finish Schedule, Public Toilet Rooms 137 and 138: delete reference to "2x2" ceiling and substitute "Exist. GWB." Existing GWB ceilings to remain and be repainted.

Item 1.07 Drawing M-101, Scope of Work Note 6: Test and Balance Services may be performed by the A/E preferred vendor, or a qualified independent Test and Balance Agency selected by the General Contractor, subject to Architect's approval.

Item 1.08 Specification Section 10520 - Fire-Protection Specialties, Paragraph 3.2: Add paragraph C, "Provide two (2) new fire extinguisher cabinets and fire extinguishers, located as directed by Architect."

Item 1.09 Specification Section 11050 - Book Depository, Paragraph 3.1.A: Change to read, "Install in locations shown on drawings. Three (3) each exterior, located in east wall of Existing Work Room 116."

Item 1.10 Specification Section 11451 - Residential Appliances: Delete. Owner will furnish and install all residential appliances.

ADDENDUM NUMBER TWO

This is a pre-qualification project requiring Bidders to be prequalified for General Construction.

Bids are due at 2:00 PM on Tuesday, November 20, 2007. The cut-off for questions to be answered by Addendum is 4:00 PM on Friday, November 16, 2007.

Item 2.01 Instructions to Bidders: Add the following:

1. Article 17.B: Permits and Associated Fees: Add, "The permit cost is estimated to be $3,900.00."

2. Article 22.4: Tests and Inspections: Add, "The Library will observe the following holidays in addition to those listed herein:

a. President's Day February 18, 2008

b. Veteran's Day November 11, 2008

c. Christmas Eve December 24, 2008"

Item 2.02 Exhibit B: Supplemental Terms and Conditions, Paragraph J: Add: "The Owner anticipates that the Contractor will receive a Notice to Proceed and will be able to commence work in the Phase 1 area on January 7, 2008. The Contractor will be permitted to mobilize on site in advance of this date, at the Contractor's option and convenience. The Owner will accept each Phase of work and issue a Partial Certificate of Substantial Completion. The Owner will accomplish all required relocation of the Library's furniture, fixtures, equipment, shelving and materials between Phases 1 & 2, 2 & 3, and 3 & 4 within seven (7) calendar days between each phase. These twenty one (21) days maximum are included within the 300 day contract time."

Item 2.03 General terms and Conditions, Article 21 - Warranty: Add: "The General Contractor's two (2) year warranty shall commence on the date of Final Completion of the entire project."

Item 2.04 Specification Section 01310 - Project Management and Coordination:

1. Paragraphs 1.4.B and C: Delete and substitute the following,

B. "The Contractor shall use the Owner's existing on site electric and water services. The Contractor shall develop a program to minimize the use of energy and water. The Contractor and all sub-Contractors shall accept the utilities as-is, where-is and shall provide all required interconnections, temporary distribution, safety devices including but not limited to GFI, back flow preventers, and other items required by governing codes and regulations."

C. "The Contractor shall make a good faith effort to minimize the use of the Owner's water and electricity and shall coordinate any required interruptions of service with the Owner in advance and at the owner's convenience."

2. Paragraph C.1: Change Frequency of meeting from "Weekly" to "Bi-monthly. The Owner and Architect reserve the right to call additional meetings should project conditions or coordination requirements warrant them."

Item 2.05 Specifications Section 01310 - Project Management and Coordination, Paragraph 1.5.B and C: Delete the phrase "full-time" before "project coordinator" in paragraph B, and "full-time" before "mechanical and electrical coordinator" and delete "shall be licensed to practice as a professional engineer in the location of the project, and." Add: "The Contractor shall provide the required coordination services listed herein using his Project Manager, Project Superintendent and/or office staff as required."

Item 2.06 Specifications Section 01322 - Photographic Documentation: Delete in its entirety.

Item 2.07 Specifications Section 01400 - Quality Control, Paragraph 1.3: Add, "The Contractor shall include the cost of Test and Balance specified in Section 15990 - Performance Verification. Any other testing shall be arranged and paid by the Owner."

Item 2.08 Specifications Section 01500 - Temporary Facilities and Controls, Paragraph 1.1: Add, "Provide temporary toilet facilities for construction workers. Contractor's personnel may not use the Library facilities. The Owner will provide any temporary facilities required for use of Library staff or Library patrons."

Item 2.09 Specifications Section 09654 - Linoleum Floor Coverings, Paragraph 3.3: Add: "D. Seams shall be heat welded. Welding rods shall be selected from full range of colors."

Item 2.10 Specifications Section 09681 - Carpet Tile for Alternate 1B - Carpeting throughout the building as depicted on Drawing A-802 as manufactured by Milliken, Inc. Add the following Product Description to Paragraph 2.1:

Item 2.13 Drawing A-101 – Site Plan: Add, "Note: General Contractor staging area will be the Existing Parking Area and ground to the east of the existing building. The Owner will relocate Dumpsters. Construction worker parking will be provided in the Existing Sod Parking Area on site."

Item 2.14 Drawing A-150 - Demolition Plan:

1. Add to Note 9, "Existing ceiling-mounted speaker system to remain. Remove and reinstall in new ceiling."

2. Add Note regarding Alternate No. 2, Men's and Women's Public Toilet Renovation, "Scope of tile removal and replacement includes replacement of existing gypsum wall board substrate with new. Use moisture resistant GWB at wet walls."

Item 2.15 Drawing A-201:

1. Conference Room 125: No structural or other preparatory work is required for the noted Future Accordion Folding Partition.

2. Existing Main Covered Entry 100: Delete Note referring to "Hurricane Netting Shutters" and substitute, "Provide removable Exterior Hurricane Panels as specified in Section 10710, secured to the exterior wall and concrete entry slab along the west exterior wall along grid line E to secure the open entry area. Secure anchors to existing steel structural members and concrete slab per manufacturer's requirements to meet Florida Building Code."

Item 2.16 Drawing A-252 - Phasing Plan: Add note, "Dust barrier shall be a temporary metal stud partition with painted gypsum wall board on the side exposed to public view. Partition shall run to underside of existing soffit or suspended ceiling. Tape joints and seal as required for dust control. Repair existing soffits upon removal."

Item 2.17 Drawing A-802, Teen's Library 134, CPT, 5, 8, 9: Add, "Allow for an equal quantity of colors 5, 8, and 9 in Teen's Library. Pattern will be provided by Architect."

Item 2.18 Drawing A-903, Finish Schedule

1. Space 106, Break Room and Finish Schedule Legend - Resilient Rubber Wall Base & Accessories: Change reference from "LF1" to "IC" and add, "IC base shall be Integral Cove Base, color to match adjacent linoleum."

2. Space 116, Work Room: Add note, "See Interior Plan Sheet A-802 for extent of Linoleum and IC base."

3. Ceilings: Add note: "All existing suspended acoustical tile and grid shall be removed and replaced."

CONTRACT AWARD

The Palm Harbor Library is happy to announce that Caladesi Construction Company of Largo, Florida, has been awarded the contract to renovate the library building in 2008. Six contractors bid on the project, and the four lowest bidders were within $44,000 of each other. The final contract amount came to $819,723. John Bratanov, who has served as a Project Manager and Superintendent in the construction industry for over 24 years, will serve as Project Manager. Mark Woodward, who has served as a Project Superintendent for 27 years, will be the Project Superintendent.

Moving services will be handled by Library Interiors of Florida, Inc., located in Tampa, Florida.
